In a nutshell: Vitamin D deficiency is associated with bone loss, poor muscle strength, falls and fracture. This information in older African Americans (AAs) is sparse. This RCT examined the effect of vitamin D on bone loss and physical performance in older AA women. This is the first study to show an association between free 25OHD and physical performance. These findings indicate a positive relationship of free 25OHD with gait speed and grip strength in older AA women. Further studies are needed to understand the role of free 25OHD.

Reducing older people’s falls in the general practice ProAct65+ trial

In a nutshell: Falls are common in older people in the UK and NHS costs associated with these falls are high. Systematic reviews suggest that home exercise and group-based interventions focusing on progressively challenging balance and increasing strength, can reduce up to 42% of falls in those with a history. ProAct65+ was a large RCT which investigated the effectiveness of a home exercise programme and a group-based exercise programme compared to usual care. Home exercise appeared less effective than group exercise, but the latter also became less effective after 24 months if the exercise did not continue.

Monthly High-Dose Vitamin D Treatment for the Prevention of Functional Decline

In a nutshell: This RCT looked into the effectiveness of high-dose vitamin D in lowering the risk of functional decline. It found that although higher monthly doses of vitamin D were effective in reaching a threshold of at least 30 ng/mL of 25-hydroxyvitamin D, they had no benefit on lower extremity function and were associated with increased risk of falls compared with 24 000 IU.

Effect of footwear on foot clearance, slippage and gait in older women

In a nutshell: Footwear is likely to be a factor in falls, a major issue affecting older adults’ health. This study investigated the effect of footwear on minimum foot clearance, heel slippage and spatiotemporal variables of gait in community-dwelling older women. The results show that older women should be supported to make footwear choices with optimal fitting features; recommendations of particular styles and features of footwear may assist during falls prevention education to reduce the incidence of foot trips and falls.

Fall rates in hospital rehabilitation units after individualised education programmes

In a nutshell: Falls are the most frequent adverse events that are reported in hospitals. The effectiveness of individualised falls-prevention education for patients were examined, supported by training and feedback for staff, delivered as a ward-level programme. Eight rehabilitation units in general hospitals in Australia participated. Individualised patient education programmes combined with training and feedback to staff added to usual care were found to reduce the rates of falls and injurious falls in older patients in rehabilitation hospital-units.

Impact of Tai-Chi on Falls Among Preclinically Disabled Older People

In a nutshell: This randomised controlled trial investigated the effectiveness of modified Sun style tai-chi on preventing falls in community-dwelling older people in Australia. The study could not support the tai-chi as a falls prevention measure among relatively well older people with modified mobility and an increased risk of disability. Low class attendance and lack of intensity of intervention may have contributed to the lack of effect.
